(2)(a) Plastic tabs, which pose a choking hazards, were inserted in some electrical outlets throughout the center. (2)(b) Internal cords which could form loops were on some window blinds throughout the center. (2)(c) Electrical cords which could form loops were accessible in both preschool rooms, on the vacuume in the school age room bathroom and in the entry area. (4)(c) A free standing floor lamp in the school age room was not secured.
